Though not much is known about Ruth, ALF tries to convince Brian to have her depicted on his birthday cake. However, Brian later chooses to have ALF depicted on his cake instead, in honor of his friend as his 7th birthday was the same time Rhonda and Skip would rescue ALF in order to bring him to a Melmacian colony.
According to ALF, Ruth was the Vanna White of Melmac, and the Betty White.
Alex Adam was known to be the one who formed Ruth the two-headed nurse's legacy and gave her the power to defend the colony.
